This evaluation form was developed to acquire necessary feedback from both parents and/or athletes regarding the 2008 LVC season. You may print more than one evaluation and complete one for the head coach and one for the assistant coach. The more feedback the LVC executive receives, the better opportunity to implement future improvements to the Club and its coaching. The executive would like to have completed forms delivered to one of its members or your team's parent rep by July 1, 2008.
